Time-restricted eating and 16:8

The most popular fasting protocol has the least impressive controlled evidence. That is worth knowing before you rearrange your life around it.

The trial that deflated 16:8

The cleanest test of time-restricted eating gave both groups the same prescribed calorie intake and varied only whether eating was confined to a window [9]. Over twelve months, weight loss was similar in both arms. Because calories were matched, the design isolates the window itself — and the window contributed little.

This matters because most positive time-restricted eating trials do not control intake. When you tell people to eat only between noon and 8pm, they eat less. The weight loss is real; attributing it to circadian biology rather than to the smaller intake is the error.

Where it does earn its place

Two randomised trials in clinical populations are more encouraging. In adults with metabolic syndrome, time-restricted eating improved outcomes against standard care [7]. In adults with type 2 diabetes, it produced weight loss in a randomised comparison [8]. And a trial combining a low-carbohydrate approach with time-restricted eating reported changes in weight and gut microbiome [6].

A systematic review of time-restricted eating combined with exercise found better body-composition and metabolic outcomes than either alone [2] — consistent with the general principle that what you do inside the window matters more than the window.

Practical notes

  • Earlier windows may beat later ones. See meal timing — this is the part of the circadian story that does hold up.
  • Protein is the constraint. Two meals makes hitting a protein target harder than three. Plan it rather than hoping.
  • Black coffee, tea and water do not break anything relevant. The metabolic definition of "breaking a fast" that matters for weight loss is calories.
  • Do not extend the window to compensate for a bad day. Longer windows have not been shown to add benefit, and adherence is the asset here.

Common questions

Is 16:8 better than 14:10?
A randomised trial compared 16:8 with 14:10 and control on weight and metabolic outcomes, and the differences between the two windows were not dramatic. The deficit matters more than the exact hours.
Does eating window length matter at all?
Only insofar as it changes how much you eat. At matched calories, the NEJM trial found essentially no independent effect [9].
Should I skip breakfast or dinner?
The circadian evidence mildly favours eating earlier, which means skipping dinner rather than breakfast. Adherence usually points the other way. See meal timing.
Is it safe long term?
For most healthy adults, yes. There are meaningful exceptions — see safety.
