Does intermittent fasting actually work?
It works. It just does not appear to work better than eating less the ordinary way, which is a much less marketable finding.
What the comparative trials show
The question that decides everything is not "does fasting cause weight loss" — restricting when you eat almost always restricts how much you eat, so of course it does. The question is whether it beats simply eating less across the whole day.
A meta-analysis addressing exactly that comparison found intermittent fasting and continuous calorie restriction produced comparable weight loss [6]. A large BMJ systematic review and network comparison of fasting strategies against each other and against control found the differences between approaches to be modest [2]. An earlier systematic review reached the same conclusion [7].

What about longer trials?
Short trials flatter every diet. A 2025 meta-analysis focused specifically on longer-term effects on body composition and cardiometabolic health in adults with overweight or obesity [1], and a 2024 analysis looked at adults with prediabetes [3]. In both, the metabolic improvements move with the weight. That is the pattern you would expect if fasting is a delivery mechanism for an energy deficit rather than an independent metabolic intervention.
Where fasting genuinely helps
- Fewer decisions. Skipping breakfast removes a meal you would otherwise have to make choices about. For a lot of people that is more sustainable than tracking every meal.
- No arithmetic. "Do not eat before noon" is a rule you can follow without a food diary. Calorie counting requires ongoing measurement that most people abandon.
- It fits some lives. Shift workers, people who are not hungry in the morning, and people who prefer one large evening meal often find a compressed window is simply how they already want to eat.
- Comparable results at lower effort is a real win. If two approaches produce the same outcome and you will stick to one of them, that one is better for you.
Where the effects are more than weight loss
Two exceptions are worth flagging. The EARLY trial tested a 5:2 meal-replacement approach for glycaemic control in adults with diabetes and reported benefits on HbA1c [8]. And a Cell Metabolism study measured brain responses to intermittent fasting versus a healthy-living diet in older adults, finding differences that are not obviously reducible to weight [9]. Neither overturns the main conclusion, but they show the question is not entirely closed.
Body-composition-focused reviews also suggest that pairing an energy deficit with exercise matters more than the pattern of the deficit [5], and fasting appears not to impair sports performance in any dramatic way [4] — covered on training fasted.
