Protocol
Alternate-day fasting: what the evidence shows
Alternating fed and fasted or very-low-calorie days
What the evidence supports
Produces weight loss in trials; the largest comparisons find no advantage over daily restriction and notably higher dropout. [1]
Adherence is the recurring problem. A protocol that works in a trial and not in a life is not a better protocol.
The finding people miss
Across the randomised literature, fasting protocols produce weight loss comparable to continuous energy restriction of the same magnitude. Where a trial equates calories, the fasting arm rarely wins. This is consistently reported and consistently under-communicated, because "it works about as well as eating less" is a poor headline.
It is still a useful result. For some people a rule about when to eat is far easier to follow than a rule about how much, and adherence is the thing that actually determines outcomes over a year.
Other protocols
- 16:8 time-restricted eating — eat within 8 hours, fast 16
- Early time-restricted eating — window shifted early, e.g. 8am-4pm
- The 5:2 diet — two heavily restricted days per week
- One meal a day (OMAD) — a single daily eating occasion
- Prolonged fasting (48 hours and beyond) — multi-day, water or minimal intake

Is it better than just eating less?
Will I lose muscle?
Who should avoid it?
